The Many Faces of PCOS: Why Every Patient’s Journey Looks Different

Posted on September 16, 2025 by Andrea Oh

Living with PCOS doesn’t look the same for everyone. Some may struggle with irregular cycles or infertility, while others face acne, hair growth, or weight changes. This wide range of symptoms can make PCOS hard to recognize, but it also underscores why every patient deserves care tailored to their unique journey. PCOS Awareness Month is a time to share these differences and empower women to seek the support they need.

Understanding the Variability of PCOS Symptoms

PCOS presents differently in each person, with women experiencing a diverse range of mild to severe symptoms. Here are some PCOS symptoms and how they can vary:

  • Menstrual irregularities: While menstrual issues are common, they can present differently. Some women may experience infrequent periods, skipped periods, or no periods at all.

  • Hormonal imbalances: PCOS is caused by a hormonal imbalance that can cause acne, excessive hair growth, and hair loss from the scalp. It's possible to experience one or more of these symptoms, or not experience them at all, at least not to the point of causing concern.

  • Polycystic ovaries: On ultrasound, the ovaries may appear enlarged and contain numerous small, underdeveloped follicles. However, having polycystic ovaries is not necessary for a diagnosis under some criteria.

  • Weight gain: PCOS can lead to weight gain and make losing weight difficult, but some women with the condition are thin.

Genetics, insulin resistance, lifestyle, chronic inflammation, and environmental factors can all influence the symptoms and severity of PCOS. These symptoms can also change with age: adolescents and women of reproductive age may experience irregular periods, acne, and excess hair, while women in menopause may face increased metabolic and cardiovascular risks.

The Importance of Individualized PCOS Care

Personalized care is essential for managing PCOS when trying to conceive due to its wide range of symptoms and the various areas it affects, including reproductive health, metabolic function, cardiovascular health, and mental well-being. Tailoring treatment plans to consider an individual's unique genetics, lifestyle, and specific health priorities can lead to better health outcomes and an enhanced overall quality of life.

PCOS Fertility Treatment Options

PCOS treatment options for those trying to conceive range from minimally invasive solutions to advanced reproductive technologies (ART). These treatments encompass a variety of approaches, catering to different needs.

  • Lifestyle changes: Exercising and eating a healthy diet can help improve ovulation and hormone balance.

  • Ovulation-inducing medications: A fertility specialist might prescribe medication to induce ovulation in patients with PCOS who are trying to conceive.

  • Intrauterine insemination (IUI): IUI may be recommended in combination with ovulation-inducing medications. This procedure involves placing prepared sperm directly into the uterus around the time of ovulation to improve chances of fertilization.

  • In vitro fertilization (IVF): IVF is a process that involves retrieving eggs, fertilizing them with sperm in a laboratory, and transferring the resulting embryos to the uterus.
